Bridgeport Hospital Joining 'Age-Friendly' Initiative

Connecticut's Bridgeport Hospital has joined approximately 100 health systems nationwide participating in the Age-Friendly Health Systems initiative. The program aims to improve healthcare for older adults, by getting 20% of U.S. hospitals and health systems involved by 2020. The John A. Hartford Foundation and the Institute for Healthcare Improvement launched the Age-Friendly Health Systems initiative in 2017, and these agencies have since partnered with the American Hospital Association and the Catholic Health Association of the United States to help hospitals and other care settings effect evidence-based interventions to better care for seniors. The program is founded on a series of practices designed to meet four essential elements of senior care: knowing and aligning care with each older adult's health outcome goals and care preferences; using age-friendly medications when necessary that do not interfere with the senior's healthcare goals and preferences; preventing, identifying, treating, and managing dementia, depression, and delirium across settings of care; and guaranteeing that older adults move safely each day.
